New Patients

We are delighted to welcome you to our practice and we are pleased that you chose us to serve your dental needs. Dedicated to the oral health of our patients, we are serious about providing superior dental care at reasonable prices and our goal is to help you feel and look your very best through excellent dental care.

Your scheduled appointment should take approximately one hour. To facilitate being seen expeditiously upon your arrival, we ask that you download and complete the Patient Registration form and the Medical History form before your arrival. Please remember to bring it with you at the time of your appointment or mail the completed form back to our office as soon as possible. Also, remember to bring any Insurance cards that might apply to you or your family when you come. We can include this information with your records.

If you are unable to make the appointment you have scheduled with us, please notify us at least 24 hours in advance.
